Collective Impact – Annual Progress Report: Concept Design or Implementation
This Annual Progress Report captures information about the progress of Collective Impact initiatives undertaken by grant recipients. It requires grant recipients to provide a high-level progress summary (including project inputs, outputs, and outcomes) in addition to detailed information on their approach, stakeholders, logic model, planned activities, and public communication efforts. The progress report also has grant recipients report on evaluation acitivities, including identifying their short-term outcomes, long-term impacts, relevant indicators, required data, and how this information will be communicated to stakeholders and the non-for-profit sector.
